If you choose to cash out, then yes there is a 20% cash out fee. Ebay/Paypal take about 10% on average, so you are paying about 10% more to have them do all of the work for you. And I'd say most of my sales on COMC have gone for 25-50% (or more) higher than what the same cards would have brought on ebay. So even paying the higher cash out % would still generally net me more money than selling on ebay.

But its your choice whether or not you cash-out and pay that 20% fee. Me personally, I've done over $6,000 in actual sales since I joined a year ago and haven't paid even a cent in cash out fees. I've spent over $5,800 back on the site on other cards and additional processing fees. But if I'd ever need the extra money, I'd have no problems paying the 20% fee as it's well worth it to sell stuff for more money on there and avoid the work.
