I just added another Heroes Leah Mangue to my collection (well, I just got the scan from Leah and it's being mailed off tomorrow. It's a sketch that was returned to her as part of the payment for doing the cards so was never inserted into a pack.

You got the basics down. It depends on the set really: For Topps it is usually six back where the artists draw their return cards first for approval then sent back once the set is released. The returns for Topps are not marked in any way to distinguish they are returns while many other companies use the phrase 'Artist Proof' to separate the two types. Based on reading some posts by the artists on these sets there have been instances where Topps actually packs out Artist returns by mistake. Topps usually makes good on these mistakes but its easy to see why that might happen.
